abstract, if applicable, should be no more than 200 words, on a separate page and double spaced; a maximum of four to six key words, from the MeSH list of the Index Medicus, should follow the abstract
personal biography, including background, academic credentials and current occupation, should be on a separate page and double spaced
all tables and figures should be provided on a separate page and labeled as described in Vancouver Style guidelines
